- He pleaded guilty to withholding documentation from an inquest in February 2019. He was the Clinical Lead at a residential home (Lancaster Lodge) in Richmond, south-west London. He left his job following a visit from inspectors at the Care Quality Commission (CQC) in March 2016. The Care Quality Commission (CQC) assessed the home as inadequate. On 2 May 2016, a 19-year-old female resident was found hanging from a bathroom door and died in hospital on 4 May 2016. He was served with a Schedule 5 notice, requiring him to attend the inquest to give evidence. Despite several reminders he failed to attend the inquest and on 1 May 2019 he was fined £650. The coroner referred the matter to the police and the Crown Prosecution Service charged him with withholding documentation in relation to a coroners inquest. On 16 August 2019, he pleaded guilty to the offence.
